Register a disability
Use this form to register your disability with BCP Council. You can also register on behalf of someone else.
You will need to:
- provide your details or the details of the person you are registering
- provide the GP details for yourself or the person you are registering
- confirm what rates of Disability Living Allowance (DLA) are being received if applicable
- confirm what rates of Personal Independence Payment (PIP) are being received if applicable
